Annandale Village is a nonprofit community supporting adults with developmental disabilities and acquired brain injuries. Our residents—known as Villagers—live with dignity, purpose, and connection across our campus.

Access to healthcare is an important part of supporting each Villager's health, independence, and quality of life. The CNA / Transportation Professional helps ensure Villagers can safely access medical appointments and healthcare services while receiving compassionate, individualized support throughout the experience.


Position Purpose

The CNA / Transportation Professional provides safe, reliable, and person-centered transportation for Villagers attending medical appointments and other healthcare services. More than providing transportation, this role supports Villagers throughout the healthcare experience by promoting comfort, communication, choice, and self-advocacy while helping ensure important information and follow-up needs are communicated to the appropriate members of the care team.

When not providing transportation, this position uses CNA skills to provide direct support to Villagers within Assisted Living and the Skilled Nursing Facility as needed.



Major Duties and Responsibilities

Providing Safe and Supportive Transportation

  • Provide safe, timely, and compassionate transportation for Villagers attending medical appointments and other healthcare-related services.
  • Safely assist Villagers with entering and exiting vehicles and ensure mobility devices and other equipment are appropriately secured.
  • Follow established vehicle, accessibility, transportation, and safety procedures.
  • Maintain assigned vehicles in a safe, clean, and ready condition and promptly report maintenance or safety concerns.
  • Coordinate transportation schedules and communicate delays, changes, or concerns to appropriate team members.


Supporting Villagers During Healthcare Experiences

  • Provide a calm, reassuring, and person-centered presence before, during, and after healthcare appointments.
  • Assist Villagers with navigating appointments and communicating their questions, preferences, needs, and concerns.
  • Promote Villager participation and self-advocacy in healthcare discussions and decisions to the greatest extent possible.
  • Respect each Villager's dignity, privacy, communication style, preferences, and individual support needs.
  • Observe and promptly communicate changes or concerns related to a Villager's health, behavior, or overall well-being.


Supporting Continuity of Care

  • Communicate relevant appointment information, provider instructions, and follow-up needs to appropriate members of the care team.
  • Ensure healthcare information and documentation received during appointments is returned to the appropriate team member.
  • Complete required transportation and appointment documentation accurately and timely.
  • Maintain Villager confidentiality and protect personal and health information.
  • Collaborate with Nursing, Assisted Living, Skilled Nursing, and other team members to support coordinated care.

Providing CNA and Direct Care Support

  • When not assigned to transportation responsibilities, provide CNA support within Assisted Living and the Skilled Nursing Facility as needed.
  • Assist Villagers with activities of daily living and other individualized needs in accordance with their plans of care, abilities, preferences, and goals.
  • Encourage Villagers to participate as independently as possible while providing the level of assistance needed for safety and success.
  • Observe and report changes in Villager health, behavior, or overall well-being.
  • Provide support in a manner that promotes dignity, privacy, choice, comfort, and quality of life.


Other Responsibilities

  • Follow infection prevention, safety, emergency response, and transportation procedures.
  • Maintain required CNA certification, driver's license, training, and competencies.
  • Participate in required meetings, orientation, and training.
  • Follow Annandale Village policies, procedures, and applicable regulatory requirements.
  • Perform other duties in the best interest of Annandale Village and the Villagers we serve.


Required Qualifications

  • Current Certified Nursing Assistant (CNA) certification in good standing and compliance with applicable Georgia and federal requirements.
  • High school diploma or GED.
  • Valid Georgia Class C Driver's License, proof of insurance, and acceptable driving record.
  • Ability to safely operate organizational vehicles and transportation equipment.
  • Ability to safely assist Villagers with mobility, transfers, and vehicle entry and exit.
  • Ability to read, understand, and follow written and verbal instructions.
  • Strong communication and interpersonal skills.
  • Ability to work independently while maintaining effective communication with the care team.


Preferred Qualifications

  • Previous CNA experience in skilled nursing, assisted living, developmental disabilities, healthcare, or a related setting.
  • Experience transporting or supporting individuals with mobility or accessibility needs.
  • Experience supporting adults with developmental disabilities or acquired brain injuries.

Employment Requirements

  • Must meet Annandale Village age and driving requirements for operating organizational vehicles.
  • Must maintain a valid Georgia Class C Driver's License, proof of insurance, and acceptable MVR.
  • Must pass GBI and FBI background checks, including fingerprinting.
  • Must pass drug screening and tuberculosis screening.
  • Must maintain current CPR and First Aid certification.
  • Must maintain CNA certification and all other required training and competencies.

Physical Requirements

  • Ability to sit and drive for extended periods while remaining attentive to Villager safety and comfort.
  • Ability to stand, walk, bend, reach, climb, kneel, and perform movements associated with transportation and direct support.
  • Ability to assist Villagers with mobility, transfers, positioning, personal care, and other individualized needs.
  • Ability to safely operate accessible transportation equipment and assist Villagers into and out of vehicles.
  • Ability to lift up to 25 pounds independently and move up to 75 pounds with assistance.
  • Ability to perform CNA, CPR, First Aid, and emergency response responsibilities as required.
