Last year, we assisted over 764,000 visitors, served over 11.7 million meals, and accessed $10.7 million in public benefits resources for our guests. Through an array of programs that we offer, we look to establish long-term stability for those we serve.

The Social Care Network Resource and Referral Case Manager will work with the various database referral platforms that include, but are not limited to, Public Health Solutions, Unite Us, Aunt Bertha, and Food Bank TEN. The objective of the position is to establish contact with existing public health and hospital systems and connect households to the appropriate services within NYCP that will provide assistance to those impacted by social determinants of health. Specific duties will include monitoring referral systems, providing a coordinated receipt of referrals, assessing the needs of the referral, connecting to appropriate NYCP services, providing follow-up for referrals both internally and externally, and maintaining appropriate reporting tools. Duties will also consist of benefits screenings and direct assistance with public benefits applications that include, but are not limited to, SNAP, TANF, Safety Net, and Rental arrears.

The Help 365 Bilingual Mobile Case Manager will report to the Social Care Network Resource and Referral Manager at the 8 East 109th, New York, NY 10029 location. The schedule for this position is Monday through Friday: 9 AM - 5 PM; 1-hour lunch. This is a Full-Time position, 35 hours a week, non-exempt. Extensive travel throughout the Bronx, Brooklyn, Manhattan, and Queens is required (an OMNY card will be provided for work-related travel).

Responsibilities:

Outreach and Partner Engagement

  • Travel throughout New York City to assigned partner sites, community locations, and outreach events to provide benefits enrollment and social care services.

  • Develop and maintain relationships with community partners and identify opportunities for expanded benefits enrollment services.

  • Manage assigned caseloads from partner sites and social care referral platforms (e.g., Unite Us, Mount Sinai clinics, Mobile Partners).

  • Provide direct enhanced services to participants in accordance with WholeYouNYC program guidelines established by Public Health Solutions and Unite Us.

  • Conduct outreach and follow-up with referred individuals and families to connect them with food assistance, public benefits, healthcare coverage, and other supportive services.

  • Accurately document all services provided using contracted service notes to ensure compliance with program requirements; ensure timely and proper case closure upon successful delivery of services.

  • Screen participants for eligibility, and assist with applications and recertifications for public benefits programs, including SNAP, Cash Assistance/TANF, One Shot Deal, SSI/SSD, Medicaid, Fair Fares, and other available resources.

  • Facilitate formal mediation between applicants and HRA as needed to secure benefits.

  • Confirm receipt and amounts of awarded benefits upon application completion.

  • Provide participants with vetted information about local community resources.

  • Elevate complex or high-priority cases to supervisors as needed for guidance and support.

  • Other responsibilities as assigned.

Qualifications:

  • A Minimum of two (2) years of experience working in Human Services or related fields is required.

  • Full professional proficiency level in English and Spanish is required.

  • A Bachelor’s degree in Social Work, Human Services, or a related field; equivalent work experience is preferred.

  • Commitment to helping the communities in need and NYCP’s work and mission.

  • Solid computer literacy and general administrative/clerical skills.

  • Professional written and verbal communication skills.

  • Proficient with Microsoft Office Suite and Google Drive.

  • Experience with case management databases.

  • Experience with HRA’s Paperless Office System (POS).

  • Experience with public benefits and utilizing database platforms to capture information.

  • Experience managing challenging interactions and de-escalating situations with clients while maintaining safety, dignity, and respect.

  • Cultural sensitivity and respect when serving diverse communities.

  • Strong multitasking skills and ability to work efficiently in a fast-paced, high-traffic environment.

  • Strong customer service and interpersonal skills.

Pay: $22.00 - $24.00/hour
