Key Responsibilities

• Conduct comprehensive reviews of clinical documentation to identify opportunities for improvement and ensure compliance with coding guidelines.
• Collaborate with physicians and interdisciplinary teams to clarify documentation and support accurate diagnosis coding (HCC coding).
• Educate healthcare providers on clinical documentation best practices, risk adjustment, and the impact of documentation on quality measures and reimbursement.
• Analyze documentation for consistency, completeness, and accuracy, making recommendations for improvement.
• Participate in ongoing training and professional development related to Clinical Documentation Integrity and Risk Adjustment.

Required Qualifications & ExperienceRequired:

• Active Registered Nurse (RN) license.
• 3+ years of experience in Clinical Documentation Integrity, Risk Adjustment, HCC Coding, or Documentation Improvement.

Preferred:

• NP license.
