Responsibilities:

• Coordinate and facilitate the admissions process for patients transitioning from acute care settings to our skilled nursing facility

• Collaborate with healthcare providers and families to ensure a smooth transition for patients

• Develop and maintain relationships with local healthcare facilities, hospitals, and physicians to generate referrals

• Conduct assessments and screenings to identify patient needs and develop personalized care plans

• Maintain accurate records and reports of admissions, transfers, and discharges

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Regional Admissions Liaison: Serve as a liaison between our facility and local healthcare providers, hospitals, and physicians to ensure effective communication and referral processes

Requirements:

• Regional Admissions Liaison: Excellent communication, organizational, and problem-solving skills

• A high school diploma or equivalent required; associates or bachelor's degree in a healthcare-related field preferred
