Home Health Field LVN/LPN

Choice Health at Home is looking for a compassionate and skilled Licensed Vocational Nurse (LVN/LPN) to join our growing Home Health team. As a Home Health LVN, you'll help patients recover, manage chronic conditions, and maintain their independence—all from the comfort of their own homes. You'll work alongside experienced clinicians in a supportive, patient-first environment where your care truly makes an impact.

What You'll Do

• Deliver skilled nursing care in patients' homes under the supervision of a Registered Nurse
• Assist with patient assessments and contribute to individualized care plans
• Administer medications and provide treatments as ordered
• Perform wound care and monitor patient progress
• Educate patients and caregivers on disease management, medications, and home care
• Maintain accurate clinical documentation and communicate effectively with the interdisciplinary care team
• Follow infection prevention and safety protocols
• Ensure high-quality, compassionate care while promoting patient independence

What We're Looking For

• Current LVN/LPN license
• Valid driver's license and reliable transportation with current auto insurance
• Current CPR certification
• Home health experience preferred
• HomeCare HomeBase experience is a plus
• Excellent communication and organizational skills
• A compassionate, patient-centered approach to care
