Position Summary

The Director of Nursing (DON) is responsible for overseeing the clinical operations of the home care agency and ensuring the delivery of high-quality, compliant patient care. The DON provides leadership and supervision to all clinical staff, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Personal Care Assistants (PCAs), and clerical personnel. This position ensures compliance with all federal, state, and agency regulations while maintaining excellent patient outcomes and staff performance.

Duties and Responsibilities
  • Manage and supervise the nursing department and all clinical personnel.

  • Recruit, train, mentor, and evaluate nursing staff.

  • Provide clinical guidance and support to field nurses and caregivers.

  • Conduct performance evaluations and address employee concerns.

  • Review, approve, and sign required clinical documentation.

  • Ensure compliance with state, federal, agency, and accreditation standards.

  • Develop, implement, and maintain agency clinical policies and procedures.

  • Conduct chart audits and quality assurance reviews.

  • Monitor patient care plans and ensure appropriate clinical interventions.

  • Collaborate with physicians, VA hospitals, case managers, and other healthcare providers.

  • Monitor and assign patient admissions, assessments, supervisory visits, and reassessments as needed.

  • Assist in the field when clinically necessary; this position is primarily administrative and is not intended for routine patient assignments.

  • Support scheduling by responding to clinical staffing issues and helping resolve urgent coverage needs.

  • Carry the agency's on-call phone outside normal business hours and respond to nursing emergencies, staff questions, and urgent clinical concerns.

  • Investigate incidents, complaints, and clinical concerns and implement corrective actions.

  • Ensure proper infection control and patient safety practices.

  • Maintain confidentiality in accordance with HIPAA regulations.

  • Participate in agency meetings, quality improvement initiatives, and regulatory surveys.

Qualifications
  • Current Registered Nurse (RN) license in good standing.

  • Previous home health or home care management experience preferred.

  • Strong leadership, communication, organizational, and problem-solving skills.

  • Knowledge of home care regulations and standards of practice.

  • Ability to manage multiple priorities and make sound clinical decisions.

  • Proficiency with electronic medical records (EMR) systems.

  • Valid driver's license and reliable transportation.

  • Compassionate, dependable, patient-centered, and committed to delivering outstanding care.

Physical Requirements
  • Ability to travel to patient homes and agency locations as needed.

  • Ability to perform occasional patient assessments and clinical visits.

  • Ability to lift, bend, and stand for extended periods when required.
