Prime Home Developmental Disability Services is a certified Nebraska DHHS provider offering residential and day services for individuals with developmental and intellectual disabilities. Through individualized programs and multiple service options, we provide a full circle of care that empowers individuals to live fulfilling lives.


  • Milestones – Abilities (Papillion): 1:1 and small group activities including music, arts & crafts, baking, and more. Hours Monday through Friday 9:00 am to 5:00 pm
  • Milestones – School (Papillion): Serving students ages 12–20, focusing on IEP goals, vocational skills, and social development through activities and community outings. Hours Monday through Friday, 7:45 am to 3:45 pm (hours may vary depending on van routes)
  • Milestones – Opportunities (Papillion): 1:1, 2:1, and small group activities with community engagement and onsite programs. Hours 7:45 am to 3:45 pm
  • Milestones – Connections (La Vista): Community outings and in-house activities for individuals and staff. Hours 9:00 am to 5:00 pm (hours may vary depending on van routes)


What You’ll Do:

  • Work 1:1 or in small groups to support individuals in achieving independence.
  • Take individuals on community outings (parks, libraries, movies, shopping, restaurants, and more).
  • Facilitate onsite activities such as cooking classes, arts and crafts, painting, and gardening in the greenhouse.
  • Provide input and assistance in planning and evaluating programs and client goals.
  • Maintain accurate documentation of client progress, incidents, and activities.



DOCUMENTATION (Yes, It Matters!)

Documentation is an essential part of a DSP's role. Accurate, timely notes help protect members, support quality care, and keep the entire team informed.

As a DSP, you are responsible for:

  • Completing daily case notes in Therap
  • Tracking behavior patterns, goals, and program participation
  • Writing clear and objective incident reports when necessary
  • Documenting what you observed, when it occurred, and how you responded
  • Recording information accurately and promptly
  • Maintaining professionalism in all written documentation

Remember: If it wasn't documented, it didn't happen. Good documentation tells the story of the member's day, supports continuity of care, and ensures compliance with program requirements.


YOUR SCHEDULE

  • Monday–Friday between (6:00 am and 5:00 pm)
  • Includes AM or PM transportation route assignment
  • No evenings, weekends, or on-call


Qualifications:

  • High school diploma or GED required.
  • Minimum age of 19 years.
  • Ability to pass a background check (criminal history and APS/CPS).
  • Strong verbal, written, and interpersonal communication skills.
  • Attention to detail and ability to meet deadlines.
  • Ability to manage stressful situations responsibly and effectively.
  • Physically capable of assisting with transfers and lifting clients who require physical support.
  • Driver's license
