RN (Geriatrics) - Full-time
Job Summary:
We are seeking a compassionate and experienced RN (Geriatrics) - Full-time to join our team at Autumnwood Care Centre, a vibrant community dedicated to providing exceptional care and support to our residents. As a key member of our nursing staff, you will play a crucial role in delivering high-quality healthcare to our geriatric patients, working closely with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities:
Provide direct patient care, including assessment, planning, implementation, and evaluation of individualized care plans for geriatric patients with complex needs
Collaborate with other healthcare professionals to develop and implement effective treatment plans, including medication management and wound care
Monitor patients' conditions, reporting any changes or concerns to the healthcare team
Educate patients and families on disease management, nutrition, and other health-related topics
Develop and maintain positive relationships with residents, families, and colleagues to promote a culture of respect, empathy, and open communication
Participate in quality improvement initiatives to continuously improve patient care and outcomes
Maintain accurate and up-to-date records of patient care and communications
