Job Summary: Performs patient care and activities In Home as directed. Reports to Senior TLC ADHC Coordinator.
Specifications

Education: High school diploma or equivalent.

Experience: At least 1 year working as a PCA, with at least 1 years’ experience working with the frail elderly population. Able to prepare simple meals, perform personal care and housekeeping tasks.

Number and Type of Employees Supervised (optional): None

Licensure, Registry or Certification Required: Valid driving license with personal vehicle insurance required. A dependable means of transportation is mandatory. Must meet competency testing requirements consistent with the appropriate occupational licensure laws and regulations.

Special Training: Good driving record; able to communicate effectively both verbally and in written form; able to lift and turn participants; knowledge of and care for adults with chronic health conditions. Only act within the scope of his or her authority to practice. Attend Senior TLC PCA training program and will meet a standardized set of competencies for the specific position description established by Senior TLC and approved by CMS before working independently.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Key Responsibilities: (*denotes an age-related skill or task)

  • Assists with tasks on a plan of care for Participants in the center and/or in the home that have been taught by the appropriate professional personnel.
  • Provides personal care to participants at Senior TLC (bathing, dressing, feeding, toileting, transfers, lifting, ambulation and incontinent care according to the participants plan of care
  • Serves meals according to participant’s specific diets.
  • Prepares simple meals according to the participant’s specific diet. Performs incidental household services which are essential to the participants care at home and according to their plan of care. Services include but are not exclusive to meal prep, housekeeping, laundry, grocery & errands in addition to personal care.
  • Assists with activities in large and small group settings.
  • Assists on outings/field trips, and appointments as needed
  • Obtains vital signs and weights per plan of care and as ordered by the physician.
  • Reports changes in medical or physical status to supervisor.
  • Assists participants with reminders of self-administration of medications which are ordered by their provider and according to their plan of care.
  • Attends staff meetings and complies with competencies per STLC policies.
  • Documents services provided and observations on appropriate forms.
  • Participates in on-call rotation for home care for weekdays and weekends.
  • Other duties as assigned by supervisor.

Participant Interface: Must be comfortable and eager to engage our participants in a warm and welcoming manner while maintaining a professional distance and attitude. Experience with issues of aging and health challenges would be helpful. Demonstrates knowledge of and supports STLC mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
