Nursing Patient Care Coordinator, RN

Are you a compassionate and organized Registered Nurse looking to make an impact in home health? We are seeking an RN Patient Care Coordinator to help lead and coordinate patient care while ensuring exceptional service, compliance, and clinical excellence.

🔹 Key Responsibilities:
• Coordinate and manage multidisciplinary patient care teams
• Schedule and oversee home health visits
• Serve as a liaison between patients, families, providers, and caregivers
• Support care plan development, updates, and compliance
• Conduct patient follow-ups and quality assurance activities
• Ensure adherence to Medicare, Medicaid, HIPAA, and home health regulations
• Collaborate with clinical and administrative teams to improve patient outcomes

🔹 Qualifications:
• Active California RN License
• Current BLS Certification
• Home Health experience preferred
• Knowledge of Medicare regulations and OASIS preferred
• Strong communication, organizational, and computer skills
• Leadership or care coordination experience preferred
