Direct Support Professional (DSP) – Community-Based Program

Salem, OR

Full-Time (40 hours/week)

At Trellis LLC, our Community-Based Program supports adults with developmental disabilities in building independence, life skills, and meaningful community connections.

We’re looking for a reliable, energetic DSP who can manage responsibility, think on their feet, and show up consistently for the people we support.

What You’ll Do

This is a fast-paced, hands-on role—not a sit-down job.

Support & Skill Development

  • Teach and model independent living and community skills
  • Provide individualized coaching until skills are mastered
  • Support clients in achieving their personal goals based on their personalized plans

Community Engagement

  • Plan and lead outings, activities, and group events
  • Transport individuals safely throughout the community (30–50 miles/day)
  • Adapt environments for sensory, mobility, and behavioral needs

Safety & Support

  • Maintain a safe environment at all times
  • Monitor health and behavioral needs (diabetes, seizures, mental health, etc.)
  • Problem-solve quickly in real-world situations

Planning & Coordination

  • Manage a caseload of up to 12 individuals
  • Coordinate schedules, routes, and activities efficiently
  • Create engaging, meaningful community experiences

Documentation

  • Track daily activities, hours, and transportation
  • Complete progress notes and reports accurately and on time
  • Follow all policies, procedures, and mandatory reporting requirements

What We’re Looking For

  • At least 1 year experience supporting individuals with disabilities (preferred)
  • Strong problem-solving and communication skills
  • Ability to manage time, schedules, and multiple individuals
  • Dependable, self-motivated, and able to work independently
  • Comfortable being active and on the go throughout the day

Requirements

  • Valid driver’s license, reliable vehicle, and insurance
  • Good driving record
  • Ability to drive 30–50 miles/day
  • CPR/First Aid Certification (or ability to obtain)
  • Ability to pass background checks and required training (including OIS)
