What You'll Do

The Case Manager provides therapeutic case management services while coordinating medical care, treatment planning, and service delivery. Working as part of a multidisciplinary team, you'll help ensure residents receive the support and resources needed to succeed.

Medical Coordination
  • Maintain residents' medical records and ensure regulatory compliance.

  • Schedule and coordinate routine, specialty, psychiatric, and follow-up appointments.

  • Assist with transportation to medical appointments when needed.

  • Monitor medication administration and complete weekly medication counts.

  • Train staff in medication administration, CPR, and First Aid.

  • Communicate medical updates with parents, guardians, and treatment team members.

Team Collaboration
  • Work closely with clinicians, direct care staff, families, and community providers.

  • Assist with program operations as needed, including providing coverage for Youth Behavioral Specialists and Supervisors.

  • Complete additional duties that support quality care and program success.

QualificationsEducation & Experience
  • High School Diploma or equivalent required

Skills & Requirements
  • Strong written and verbal communication skills.

  • Ability to maintain accurate documentation and electronic records.

  • Critical thinking and problem-solving skills.

  • Ability to complete required trainings, including CPR/First Aid, Medication Administration, Crisis Intervention, Trauma-Informed Care, HIPAA, Mandated Reporting, and related certifications.

  • Ability to lift up to 25 pounds and meet the physical requirements of the position.
