What You'll Do
The Case Manager provides therapeutic case management services while coordinating medical care, treatment planning, and service delivery. Working as part of a multidisciplinary team, you'll help ensure residents receive the support and resources needed to succeed.
Medical CoordinationMaintain residents' medical records and ensure regulatory compliance.
Schedule and coordinate routine, specialty, psychiatric, and follow-up appointments.
Assist with transportation to medical appointments when needed.
Monitor medication administration and complete weekly medication counts.
Train staff in medication administration, CPR, and First Aid.
Communicate medical updates with parents, guardians, and treatment team members.
Work closely with clinicians, direct care staff, families, and community providers.
Assist with program operations as needed, including providing coverage for Youth Behavioral Specialists and Supervisors.
Complete additional duties that support quality care and program success.

QualificationsEducation & Experience
High School Diploma or equivalent required
Strong written and verbal communication skills.
Ability to maintain accurate documentation and electronic records.
Critical thinking and problem-solving skills.
Ability to complete required trainings, including CPR/First Aid, Medication Administration, Crisis Intervention, Trauma-Informed Care, HIPAA, Mandated Reporting, and related certifications.
Ability to lift up to 25 pounds and meet the physical requirements of the position.
