ESSENTIAL FUNCTIONS:
The Community Response Worker is on the frontline connecting healthcare, government, and other social service systems with Chicago communities. The incumbent will facilitate access to resources and service delivery to improve quality and cultural competence. The Community Response Worker will increase individual and community health knowledge and self-sufficiency through a range of activities such as outreach, community education, and resource navigation.
POSITION RESPONSIBILITIES:
- Comply with all CDPH and Community Response Corps training regarding confidentiality and the handling of personally identifiable information; and follow all scripts, policies, and procedures established for the program.
- Conduct outreach to individuals regarding potential exposure, self-quarantine procedures, symptom management, and referrals for testing and other resources.
- Communicate professionally and empathetically with colleagues, supervisors, and the public.
- Collect and record information systematically in a centralized database.
- Collaborate with the supervisor to identify and achieve project goals.
- Participate in professional development, trainings, and weekly team meetings.
MINIMUM QUALIFICATIONS:
- High School or GED required.
- Excellent interpersonal, verbal and written skills.
- Intermediate level computer skills, Microsoft software programs, email accounts, web browsers, as various databases.
- Applicants must reside in the City of Chicago.
- Must be 18 years of age or older.
- Prior justice involvement background check results will not automatically disqualify a candidate.
- Proof of identity and authorization to work in the United States (I-9 documentation) is required.
