Specialized Caregiver Service Description

Purpose

The Specialized Caregiver provides advanced non-medical personal care and support for clients with complex physical or cognitive conditions requiring a higher level of experience, observation, and compassionate care. These caregivers are selected based on their experience, additional training, and ability to care for clients with specialized needs while promoting safety, dignity, comfort, and independence.

Specialized Caregivers perform all Companion Care and Personal Care Assistant (PCA) services, while providing additional support for clients with chronic illnesses, advanced dementia, hospice support, Parkinson's disease, stroke recovery, cancer, and other high-acuity conditions.

Specialized Care Services

Advanced Personal Care

  • All Companion Care and PCA services

  • Complex transfers and mobility assistance using approved equipment

  • Bed-bound client care and frequent repositioning

  • Assistance with advanced incontinence care

  • Skin observation and reporting concerns to the office

  • Assistance with pressure injury prevention through repositioning and skin care

Dementia & Alzheimer's Care

  • Provide compassionate care for clients with memory impairment

  • Utilize redirection and de-escalation techniques

  • Maintain structured daily routines

  • Reduce environmental triggers that may increase confusion or anxiety

  • Encourage cognitive stimulation through appropriate activities

  • Monitor for wandering risks and maintain client safety

  • Provide reassurance during periods of confusion or agitation

Chronic Illness & High-Acuity Support

Assist clients living with conditions such as:

  • Alzheimer's disease and other dementias

  • Parkinson's disease

  • Stroke recovery

  • Multiple sclerosis (MS)

  • ALS (Lou Gehrig's Disease)

  • Cancer

  • COPD

  • Heart failure

  • Diabetes

  • Hospice and end-of-life support

  • Other chronic or progressive illnesses requiring enhanced assistance

Mobility & Safety

  • Fall prevention interventions

  • Safe ambulation assistance

  • Proper body mechanics during transfers

  • Wheelchair assistance

  • Monitoring for changes in mobility and endurance

  • Assistance with prescribed home exercise programs, as directed by therapy professionals

Nutrition & Hydration

  • Meal planning and preparation

  • Feeding assistance when appropriate

  • Monitor food and fluid intake

  • Encourage proper hydration

  • Report changes in appetite or swallowing concerns to the office

Family Support

  • Provide emotional support to clients and family members

  • Reinforce established care plans

  • Communicate observations and concerns to the office promptly

  • Assist families in maintaining a calm, supportive environment

Documentation

  • Accurately document all care provided

  • Report changes in physical, cognitive, emotional, or environmental conditions immediately

  • Follow all care plans, agency policies, HIPAA regulations, infection prevention practices, and safety standards

Specialized Caregiver Service Limitations

Although Specialized Caregivers provide care for clients with higher-acuity needs, they remain non-medical caregivers and do not perform skilled nursing services.

Specialized Caregivers do not:

  • Administer medications or injections

  • Perform sterile wound care or dressing changes

  • Insert or irrigate Foley catheters

  • Perform tracheostomy care

  • Administer tube feedings

  • Start or manage IV therapy

  • Adjust oxygen settings

  • Perform skilled nursing assessments

  • Accept physician orders

  • Perform any task requiring a licensed healthcare professional

Specialized Caregivers may assist with routine, non-skilled support around existing medical equipment (such as emptying a Foley catheter drainage bag or providing observation and reporting), in accordance with state regulations, agency policy, and the client's care plan.


Reliable transportation is required.
