DAY IN THE LIFE AS A SOCIAL SERVICES ASSISTANT

As a Social Services Assistant for our elder care facility, you will be asked to assist with coordinating discharge planning, care plan meetings, post-discharge follow-up, etc. You will also coordinate staff training sessions such as Residents’ Rights, Guest Relations and Resident Dignity. Most importantly, you go home knowing you made a difference in the lives of others.

QUALIFICATIONS

• Previous experience in a long term care facility

• Knowledge of discharge planning and DME is a plus
