What Youll Need:
- Minimum age: 18
- High school diploma or equivalent
- Strong communication, organization, and problem-solving skills.
- Proficiency in Microsoft Office and Therap.
- Valid drivers license and Social Security number
- Basic computer and documentation skills
- Ability to pass required background and reference checks
About the Role:
The Employment Support Professional is responsible for supporting individuals with disabilities in achieving meaningful employment through person-centered, community-based services. This role focuses on helping individuals build employment skills, explore career interests, and move toward competitive, integrated employment. Services are tailored to each individuals goals, strengths, and support needs.
Your responsibilities may include:
- Professional Responsibilities
- Foster a person-centered environment that promotes growth, independence, and skill development.
- Build positive and respectful relationships with individuals supported, coworkers, community partners, families, and stakeholders.
- Communicate concerns, incidents, and recommendations to supervisors in a timely and professional manner.
- Attend required trainings, meetings, and professional development opportunities.
- Adhere to organizational policies, including dress code, safety standards, and timekeeping requirements.
- Maintain accurate attendance and time records using approved systems.
- Engage individuals in meaningful, person-centered interactions throughout each shift.
- Implement individualized employment and vocational training plans based on personal goals and interests.
- Conduct discovery activities to identify strengths, interests, skills, and preferred employment conditions.
- Develop and customize training strategies that align with each individual's abilities and career objectives.
- Facilitate soft skills training, job readiness activities, and community exploration opportunities.
- Provide ongoing coaching, encouragement, and feedback to support employment readiness and skill development.
- Ensure work areas, tools, and supplies are maintained and secured at the end of each shift.
- Perform department-specific duties and responsibilities as assigned.
- Provide direct support in employment skills training and daily living activities, including:
- Dining and meal support
- Grooming and personal care
- Self-help skills
- Toileting assistance
- Self-advocacy development
- Medication administration
- Medical support
- Behavioral support
- Community participation and engagement
- Encourage participation in meaningful activities based on each individual's preferences, needs, and goals.
- Support individuals in developing self-advocacy and independent decision-making skills.
- Utilize technology and monitoring systems as required to ensure safety, engagement, and appropriate supervision.
- Follow all Bloodborne Pathogen and Infectious Disease Prevention procedures.
- Complete all required training and certification requirements by established deadlines.
- Maintain accurate and timely documentation in Therap and other required systems.
- Document training programs, skill development activities, attendance, progress notes, and accountability records.
- Record medical, behavioral, and significant event information by the end of each shift.
- Monitor and respond to Therap To-Do lists, T-Logs, and SComms daily.
- Complete incident reports and written statements related to critical incidents or allegations as required.
- Ensure all documentation is clear, accurate, and completed according to agency standards.
- Follow emergency documentation procedures when electronic systems are unavailable.
- Support individuals at community employment sites, enclaves, and vocational training locations.
- Provide supervision, coaching, and assistance to individuals working in employment settings.
- Assist with transportation, route support, and worksite coverage as needed.
- Promote successful work experiences by encouraging productivity, independence, and workplace skills.
- Assist in providing coverage across multiple locations and service programs as needed.
- Accept reassignment to employment, community-based, or supportive living services during staffing shortages, inclement weather, or program closures.
- Support organizational efforts to ensure continuity of services for all individuals receiving support.
- Perform other duties as assigned.
Physical Requirements:
This role involves regular standing, walking, lifting (up to 50 lbs), and hands-on support. Reasonable accommodations are available.
Work Environment
- Community Employment Services day programs classrooms setting, includes job sites, businesses, and community-based training environments
- Supportive Living- homes and apartments where people supported live.
- Will access the community with people supported on a regular basis
- All programs have frequent use of computer and/or laptop, household appliances, Center-owned vehicles, Center-owned cell phones, assigned adaptive equipment that supports the person.
Travel
Travel may be required for training when applicable
