Baywood Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate care, support patients and families, and help individuals maintain their independence in the comfort of their own homes. What You'll Do

  • Conduct patient assessments and develop individualized care plans.
  • Coordinate services with physicians, therapists, caregivers, and community resources.
  • Monitor patient progress and adjust care plans as needed.
  • Educate patients and families on medications, treatment plans, and wellness strategies
  • Maintain accurate clinical documentation and regulatory compliance.

Qualifications

  • Current Minnesota Registered Nurse (RN) license , required.
  • Home health, case management, or transitional care experience, required.
  • Strong assessment, communication, and care coordination skills.
  • Must be 21 years of age or older.
