Personal Caregiver Assistant
Purpose
The Personal Care Assistant (PCA) provides compassionate, non-medical, hands-on assistance to clients who require support with activities of daily living (ADLs). The goal of PCA services is to promote safety, dignity, independence, and comfort while allowing clients to remain in the familiar surroundings of their own home.
In addition to personal care, PCAs provide companionship, emotional support, and assistance with routine household tasks to enhance the client's overall quality of life.
Personal Care Services
Personal Care Assistants may provide assistance with:
Activities of Daily Living (ADLs)
Bathing, showering, sponge baths, and personal hygiene
Dressing and grooming
Oral care
Hair care (excluding cutting or chemical treatments)
Shaving with an electric razor
Toileting and continence care
Incontinence care and changing briefs
Assistance with bedpan, urinal, and bedside commode
Mobility assistance
Transfers and ambulation assistance
Repositioning to promote comfort and skin integrity
Assistance with range-of-motion exercises as directed

Feeding assistance when appropriate
Nutrition & Meal Support
Meal planning and preparation
Assistance with feeding and hydration
Monitoring food and fluid intake when requested
Meal setup and cleanup
Companion Care Services
Meaningful companionship and conversation
Emotional support and encouragement
Participation in hobbies, games, reading, and recreational activities
Accompaniment on walks and outings
Transportation to appointments, shopping, church, and community activities (when authorized)
Household Assistance
Laundry and linen changes
Washing dishes
Light housekeeping
Vacuuming, dusting, sweeping, and mopping
Organizing commonly used living areas
Grocery shopping and errands
Prescription pickup (excluding controlled substances unless specifically authorized)
Safety & Wellness
Medication reminders only (no medication administration)
Observe and report changes in the client's physical, emotional, cognitive, or
environmental condition
Promote a safe home environment and reduce fall risks
Encourage client independence while providing appropriate supervision
Documentation
Accurately document services provided during each visit
Promptly report concerns, incidents, or changes in condition to the office

Follow all SYNERGY HomeCare policies, HIPAA regulations, infection prevention
practices, and safety standards
PCA Service Limitations
Personal Care Assistants provide non-medical care and do not perform skilled nursing services or medical procedures. The following tasks are outside the PCA scope of practice unless specifically authorized by state regulations and agency policy:
Medication administration or injections
Wound care or dressing changes
Foley catheter insertion or sterile catheter care
IV therapy
Tracheostomy care
Tube feedings
Oxygen adjustment
Skilled assessments
Taking physician orders
Any task requiring a licensed nurse or other healthcare professional
Reliable transportation is required.
