Locums Post Acute SNF Physician- Denver, CO

Specialty:

Coverage Needs:
- Need ASAP- Ongoing
- 3 month minimum commitment requested
- Open to Locums to Perm
Shifts:
- Monday - Friday
- Can be flexible on hours
- Must be able to work at least 30 hours per week
Setting:
- Skilled Nursing Facility
- 100% Senior Care
- Ages 65+
- Medicare Advantage experience is preferred
Census:
- 12-18 pts per day
Support Staff:
- Full support staff
Procedures:
- All bread and Butter primary care
- Clinical model designed to reduce ER visits and admissions
- Will move between facilities in Denver metro
Board Certification:
- Board Certified Required
Travel:
- Facility will cover
Credentialing timeline:
- Expedited Onboarding hoping for ASAP start
Requirements:
- Active COstate license
- Active DEA in COor willing to transfer
- BLS
