Crossroads Family Care is seeking a compassionate and dedicated **Family Nurse Practitioner (FNP)** to join our primary care team in East Texas. As a Federally Qualified Health Center (FQHC), Crossroads is committed to expanding access to high-quality, affordable healthcare and improving the health of the communities we serve.
The Family Nurse Practitioner will provide comprehensive primary and preventive care to patients across the lifespan. This is an excellent opportunity for a provider who values meaningful patient relationships, collaborative care, and the opportunity to make a difference in a community-based healthcare setting.
What You'll Do
* Provide comprehensive primary care to patients across the lifespan
* Perform examinations, assessments, diagnoses, and treatment planning
* Manage acute and chronic health conditions
* Order and interpret appropriate diagnostic tests and screenings
* Prescribe medications and treatments within scope of practice
* Promote preventive care, wellness, and patient education
* Collaborate with medical, dental, behavioral health, and other members of the care team
* Maintain timely and accurate clinical documentation
* Participate in quality improvement initiatives that support positive patient outcomes
Qualifications
* Master's or doctoral degree in nursing from an accredited program
* Current Texas APRN licensure or eligibility for Texas licensure
* Current national certification as a Family Nurse Practitioner
* Prescriptive authority in accordance with Texas requirements
* Current BLS certification
* Strong clinical judgment, communication, and interpersonal skills
* Commitment to providing compassionate, patient-centered care to diverse populations
