Position: Wound Care Nurse (RN)
Location: Brooklyn, NY
Shift: Full-Time | 6:00 AM – 2:00 PM
Salary: $110,000 annually

Job Summary

Seagate Rehabilitation and Nursing Center is seeking a dedicated Wound Care Nurse (RN) to join our team. The Wound Care Nurse will play a vital role in the assessment, treatment, and prevention of wounds, ensuring the highest standards of resident care. This role requires a detail-oriented professional who can work independently while collaborating closely with the nursing leadership team.

Key Responsibilities
  • Perform comprehensive rounds on all facility floors to identify and monitor skin alterations.

  • Capture and document photos of all wounds/skin changes.

  • Initiate wound care orders and ensure proper treatments are administered.

  • Complete weekly Medicare notes for residents on floors 1 through 4, in accordance with guidelines.

  • Maintain and update Section M of the MDS for all residents with wounds.

  • Update Braden scores quarterly or as conditions change.

  • Develop, initiate, and update care plans for alterations in skin integrity.

  • Conduct pressure ulcer investigations for nosocomial sites and track wound trends.

  • Collaborate with the ADNS on interventions to address wound care trends.

  • Communicate promptly with Unit Managers regarding wound-related concerns.

  • Ensure wound care equipment and supplies are readily available.

  • Perform daily assessments of residents pre- and post-nebulizer treatments, documenting findings.

  • Participate in quality assurance audits and data collection for wound statistics.

  • Function independently while upholding facility policies and best practices.

  • Perform other duties as assigned by the Director of Nursing, especially in emergencies.

Qualifications
  • Current and valid RN license in the State of New York.

  • Minimum of 6 months of experience in rehabilitative and restorative nursing practices required.

  • 2 years of supervisory experience in a hospital, skilled nursing facility, or related healthcare setting preferred.

  • Strong assessment, documentation, and communication skills.

  • Ability to work both independently and collaboratively in a fast-paced environment.
