Job Description
Lakeland Nursing & Rehabilitation in Haskell, NJ is hiring a full-time Nurse Practitioner (NP). Lakeland is a 201-bed skilled nursing and rehabilitation center in Passaic County that has been serving the Haskell community for years, known for a warm, homelike atmosphere and a rehab program that runs seven days a week with a state-of-the-art therapy gym. As our Nurse Practitioner, you'll work alongside a team of in-house, board-certified specialists managing everything from short-term rehab patients to long-term care residents. This is a great fit for a Nurse Practitioner who wants variety in their caseload and a supportive, established clinical team around them.
We're looking for a Nurse Practitioner (NP) with at least 6 months of NP experience. Nursing home experience is preferred — an NP who has worked in a skilled nursing or long-term care setting will hit the ground running here.
What You'll Do
Provide direct patient care and medical management across short-term rehab and long-term care residents
Perform assessments, manage chronic and acute conditions, and update treatment plans in collaboration with attending physicians
Round on residents and respond to changes in condition
Communicate with residents, families, and nursing staff to keep care coordinated
Qualifications
Active NJ Nurse Practitioner (NP) / Advanced Practice Nurse (APN) license
Minimum 6 months of experience as a Nurse Practitioner (NP)
Experience in a skilled nursing facility (SNF) / long-term care (LTC) setting preferred
