About the Role:

The MCS Case Manager (Multi-Lingual) plays a critical role in coordinating and managing comprehensive care services for clients within diverse communities across the United States. This position involves assessing client needs, developing individualized care plans, and facilitating access to appropriate resources and support systems. The Case Manager acts as a liaison between clients, healthcare providers, and community organizations to ensure seamless service delivery and positive outcomes. Multilingual capabilities are essential to effectively communicate with clients from various linguistic backgrounds, enhancing engagement and trust. Ultimately, this role aims to improve client well-being by providing culturally sensitive, personalized case management and advocacy.

Minimum Qualifications:

  • Bachelor’s degree in Social Work, Nursing, Psychology, or a related field.
  • At least two years of experience in case management or a related healthcare/social services role.
  • Proficiency in English and at least one additional language commonly spoken in the United States.
  • Strong knowledge of community resources, healthcare systems, and social service programs.
  • Excellent organizational, communication, and interpersonal skills.

Preferred Qualifications:

  • Master’s degree in Social Work (MSW) or related discipline.
  • Certification in Case Management (CCM) or equivalent credential.
  • Experience working with diverse populations, including immigrant and refugee communities.
  • Familiarity with electronic health records (EHR) and case management software.
  • Additional language proficiencies beyond the minimum requirement.

Responsibilities:

  • Conduct thorough assessments of clients’ medical, social, and psychological needs to develop tailored care plans.
  • Coordinate and monitor services by collaborating with healthcare providers, social services, and community resources.
  • Maintain accurate and up-to-date case records, documenting client progress and service delivery.
  • Provide direct support and advocacy for clients, ensuring they understand and can access available services.
  • Utilize multilingual skills to communicate effectively with clients and stakeholders, overcoming language barriers.
  • Identify and address potential barriers to care, including cultural, linguistic, and socioeconomic factors.
  • Participate in interdisciplinary team meetings to discuss client cases and optimize care strategies.
  • Educate clients and their families about health management, community resources, and self-advocacy.
