Hammonton Center for Rehabilitation and Healthcare is hiring a full-time Nurse Practitioner (NP) to join our clinical team in Hammonton, NJ — and we're moving fast. This Nurse Practitioner (NP) opening is immediate, and we're scheduling interviews right away.

What You'll Do
Perform assessments and manage the ongoing medical care of residents in our skilled nursing and rehabilitation units
Diagnose and treat acute and chronic conditions, ordering and interpreting labs and diagnostics as needed
Collaborate with attending physicians, nursing leadership, and the interdisciplinary care team on treatment plans
Document visits and maintain accurate, timely clinical records
Communicate with residents and families about care plans and changes in condition
Qualifications
Active Nurse Practitioner (NP) / Advanced Practice Nurse (APN) license in New Jersey
Minimum 6 months of experience as a Nurse Practitioner (NP)
Experience in a nursing home / Skilled Nursing Facility (SNF) / Long-Term Care (LTC) setting preferred
