Home Health Aide

Job Description: Delivers quality home health care services to the Agency’s clients under the direction of a Registered Nurse or Therapist in accordance with physician orders, Agency and industry standards, and federal and state regulations.

Reports to: Registered Nurse or Therapist and Staffing Coordinator

Essential Duties and Responsibilities:
)81; Assists residents with activities of daily living, including bathing, dressing, grooming, toileting, eating, transferring, and mobility
)81; Engages residents in meaningful conversation, socialization, and activity while providing personal care assistance
)81; Responds to all resident needs and/or requests for care promptly, and provides or obtains assistance with care as necessary
)81; Observes residents for changes in physical, emotional, mental, or behavioral condition, unusual symptoms, accidents, injuries, or unusual occurrences, and reports promptly to a licensed nurse
)81; Completes all forms and documentation such as incident reports, change of condition reports, and behavior tracking, as requested, and in a timely manner
)81; Prepares and serves meals, assists with eating, not including tube feeding
)81; Makes beds, empties garbage, and tidies rooms daily and as needed

Education and Experience:
)81; Washington DC Home Health Aide license
)81; CPR Certificate
)81; High school diploma or equivalent
