Advantage Hospice is a physician-owned, locally operated, patient-focused, Medicare-certified and CHAP-accredited hospice agency serving patients and families throughout Northeast Ohio.
As our agency continues to grow, we are seeking an experienced hospice RN to serve as our Clinical Manager / Clinical Educator / Director of Clinical Training. This position is ideal for a knowledgeable hospice nurse who enjoys mentoring clinicians, strengthening documentation, developing training programs, and helping employees provide consistent, compliant, compassionate end-of-life care.
Working closely with the Director of Nursing, Administrator, Medical Director, and interdisciplinary team, this individual will oversee clinical education, staff development, competency validation, documentation improvement, and day-to-day clinical support. The position will also assist with chart audits, quality initiatives, orientation, and field supervision.

What Youll Do
- Develop, coordinate, and deliver clinical orientation and ongoing education for RNs, LPNs, STNAs, and other interdisciplinary team members.
- Create structured orientation plans for newly hired clinical employees and monitor their progress throughout the onboarding period.
- Provide field training, mentoring, and clinical support to nurses and hospice aides.
- Evaluate employee knowledge and clinical competency through observation, skills validation, return demonstrations, chart review, and follow-up education.
- Educate staff regarding hospice philosophy, eligibility, levels of care, symptom management, care planning, documentation, and interdisciplinary coordination.
- Provide training on Medicare Hospice Conditions of Participation, Ohio requirements, CHAP standards, agency policies, and documentation expectations.
- Assist clinical employees with accurate, timely, and complete documentation that supports the patients condition, plan of care, hospice eligibility, quality of care, and reimbursement.
- Conduct routine and focused chart audits and provide individualized coaching based on identified findings.
- Identify documentation trends, knowledge gaps, and recurring clinical concerns that require additional education.
- Develop annual education plans, in-services, competency programs, training materials, and remediation plans.
- Maintain organized records of completed education, competencies, licenses, certifications, and required training.
- Provide education and support related to hospice admissions, recertifications, HOPE assessments, symptom follow-up visits, transfers, live discharges, revocations, and death visits.
- Assist with case review and provide clinical guidance to field staff regarding changes in condition, symptom management, and coordination of care.
- Participate in IDG meetings and help ensure that documentation and plans of care reflect the patients current needs.
- Participate in QAPI and performance improvement activities by helping translate audit findings and quality data into focused staff education.
- Assist with clinical policy review and the implementation of new regulatory, documentation, and operational requirements.
- Collaborate with the Director of Nursing on employee coaching, performance improvement, and corrective education.
- Conduct supervisory and patient visits in homes, assisted living communities, and skilled nursing facilities as needed.
- Maintain clinical competence and assist with patient care or coverage when operationally necessary.
- Promote a positive, accountable, learning-focused culture in which employees feel supported while maintaining high clinical standards.
- Serve as a clinical resource for staff while appropriately escalating patient-care, compliance, and performance concerns to the Director of Nursing.

The ideal candidate is organized, patient, detail-oriented, comfortable speaking in front of groups, and able to translate complex hospice requirements into practical guidance for clinicians in the field.
Preferred experience includes:
- Hospice clinical care and operations

Required Qualifications
- Graduate of an accredited school of nursing
- Current Ohio Registered Nurse license in good standing
- Minimum of two years of nursing experience in hospice, home health, palliative care, or skilled home care
- Direct hospice experience strongly preferred
- Previous experience as a clinical educator, preceptor, supervisor, clinical manager, assistant director of nursing, or experienced RN case manager preferred
- Working knowledge of Medicare hospice regulations, documentation requirements, the IDG process, QAPI, and plans of care
- Strong clinical assessment, teaching, communication, organization, and documentation skills
- Ability to provide education to individuals and groups using different learning approaches
- Ability to coach employees professionally while maintaining appropriate accountability
- Proficiency with electronic medical records and standard office technology
- Ability to travel to patient homes and facilities throughout the agencys service area
- Valid drivers license, reliable transportation, and automobile insurance
Physical Requirements
- Ability to perform patient assessments and provide hands-on patient care when needed
- Ability to demonstrate clinical procedures and evaluate employee competencies
- Ability to access patient homes, assisted living communities, and skilled nursing facilities
- Ability to sit, stand, walk, bend, stoop, and reach as required
- Ability to occasionally lift or carry up to 30 pounds
