Isoke Developmental Services is seeking a compassionate Licensed Practical Nurse (LPN) to support an adult individual receiving In-Home and Community Support (IHCS) services. This position supports one adult individual in a structured home and community setting, providing skilled nursing support and daily assistance as part of a 2:1 care team. The role offers a stable daytime schedule and the opportunity to build a meaningful relationship while helping the individual maintain health, independence, and community engagement.
What You'll Do
• Provide nursing oversight and health monitoring in accordance with the Individual Support Plan (ISP)
• Administer medications and support G-Tube feeding and health protocols
• Monitor vital signs and report health or behavioral changes
• Assist with Activities of Daily Living (ADLs) including hygiene, dressing, and mobility support
• Support the individual during community activities and social engagement
• Accompany the individual to medical appointments when required
• Support behavioral strategies using trauma-informed approaches
• Document services through the EVV documentation system
• Communicate with physicians, support coordinators, behavioral specialists, and family members
Qualifications
• Active Pennsylvania LPN License
• CPR/BLS Certification
• Experience in home care, IDD services, behavioral health, or private duty nursing preferred
• G-Tube experience preferred – training available
• Comfortable supporting individuals in community settings and appointments
• Reliable transportation
