We are seeking a compassionate and motivated Physical Therapist (PT) to join our growing home health team! In this role, you will help patients regain mobility, reduce pain, restore independence, and improve overall quality of life—all from the comfort of their homes.

What You’ll DoAs a Home Health Physical Therapist, you will:
- Perform and document comprehensive initial evaluations and develop individualized treatment plans
- Assess patient needs and identify functional limitations prior to intervention
- Design and implement rehabilitative programs to improve mobility, strength, balance, and pain management
- Evaluate patient progress and modify treatment plans to maximize outcomes
- Educate patients and caregivers on exercises, mobility strategies, and safety techniques
- Maintain accurate and timely clinical documentation
- Collaborate with physicians, nurses, and interdisciplinary care teams
- Follow infection control and universal precaution standards during patient care
- Support agency philosophy, mission, and quality care standards
- Graduate of an accredited Physical Therapy program
- Current and active Physical Therapist license
- Minimum 1 year of home health experience preferred
- CPR Certification required
- Valid driver’s license and reliable transportation
- Current auto insurance
- HomeCare HomeBase experience preferred
- Ability to stand, walk, sit, bend, squat, kneel, and climb for extended periods
- Ability to lift, carry, push, or pull up to 100 lbs.
