Client Care Coordinator

Do you have a passion for leadership, organization, and making a difference in the lives of others? We're looking for a Client Care Coordinator to help deliver exceptional care to our clients while supporting and developing our caregiving team.

This is a unique opportunity to combine hands-on caregiving with an office leadership role in a growing home care agency.

• Pay: $24–$25 per hour

• Schedule: 40 hours per week

  • 20 hours providing caregiving services

  • 20 hours serving as Client Care Coordinator

• Conduct client quality assessments and caregiver quality reviews

• Coordinate caregiver orientations

• Support caregiver engagement, retention, and performance

• Conduct in-home assessments with prospective and current clients.

• Evaluate clients' physical, cognitive, emotional, and social care needs.

• Develop individualized care plans based on assessment findings.

• Explain SYNERGY HomeCare services and answer client and family questions.

• Collaborate with the office team to coordinate caregiver matching and service start dates.

• Document assessments accurately and maintain confidential client records.

• Monitor changes in client conditions and recommend updates to care plans as needed.

• Build positive relationships with clients, families, healthcare professionals, and community partners.

• Ensure compliance with company policies and applicable state regulations.

• Help foster a positive, supportive workplace culture

• Participate in the on-call rotation as needed

• High school diploma or equivalent

• Previous supervisory or management experience (home care preferred)

• HR knowledge is a plus

• Strong organizational, communication, and customer service skills

• Proficiency with Google Workspace

• Experience with AxisCare, CRM systems, and Care Academy is preferred but not required

• Self-motivated, dependable, and able to work independently
