Job Description
The Home Health Aide provides personal care and health-related services to patients in their homes, supporting their independence and quality of life under the supervision of nursing staff.
Qualifications
- Completion of a state-approved Home Health Aide training program
- High school diploma or equivalent (preferred)
- Valid driver's license and reliable transportation
- Current CPR and First Aid certification
- Ability to read, write, and communicate effectively in English
- Physical ability to perform essential job functions
Essential Duties and Responsibilities
- Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
- Perform light housekeeping duties related to patient care
- Prepare and serve meals according to prescribed diets
- Assist with medication reminders (as allowed by state regulations)
- Monitor and record patient's condition, reporting changes to the supervising nurse
- Measure and record vital signs
- Assist with range of motion exercises and ambulation
- Provide emotional support and companionship to patients
- Maintain a safe and clean environment for the patient
- Document care provided in accordance with agency policies and procedures
- Maintain patient confidentiality and adhere to HIPAA regulations
- Participate in care planning meetings as requested
