LPN 2nd Shift

Adult Community Training (ACT) is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to support four adults in a residential home setting in Lenoir City. This position plays a key role in ensuring the health, safety, and overall well being of the individuals we support while working alongside Direct Support Professionals (DSPs).

Schedule:
Monday through Friday, 4:00 PM to 12:00 AM
Consistent weekday schedule with no rotating shifts

What You’ll Do:

• Provide direct nursing care and oversight for four individuals in a home setting

• Administer medications and treatments as prescribed

• Monitor health conditions and respond to changes or concerns

• Complete nursing documentation accurately and in a timely manner

• Communicate effectively with DSP staff, supervisors, physicians, and other members of the care team

• Provide guidance and support to DSPs related to medical needs and care plans

• Assist with appointments, follow ups, and coordination of care as needed

• Ensure compliance with all state regulations and ACT policies

What to Expect in This Role:

• This is a hands on role in a home environment, not a clinical or hospital setting

• You will be working closely with the same individuals each day, building consistency and familiarity

• The pace can vary depending on the needs of the individuals, including times that require quick response and critical thinking

• Teamwork with DSPs is essential, as they provide daily support and rely on nursing direction for medical care

Qualifications:

• Current LPN license in the state of Tennessee

• Experience working with individuals with intellectual or developmental disabilities is preferred, but not required

• Strong communication and documentation skills

• Ability to work independently while also being part of a team

• Reliable and consistent attendance
