Position Summary:
4 Your Peace of Mind, LLC is seeking a compassionate and reliable Licensed Practical Nurse (LPN) for a private duty Per Diem assignment in Bolton, CT. The LPN will provide direct, patient-centered care under RN supervision in accordance with the Connecticut Nurse Practice Act and agency policies.

Qualifications:

• Active, unrestricted Connecticut LPN license
• Graduate of an accredited practical nursing program
• 1+ year of home health or related experience preferred
• Reliable vehicle, valid driver’s license, and auto insurance
• Strong communication, documentation, and time management skills
  
  

Responsibilities:

• Provide hands-on patient care and education
• Administer medications and treatments per orders
• Document all visits and MARs accurately within 24 hours
• Report any changes in patient condition to supervising RN
• Adhere to infection control and safety protocols
• Use EVV system for visit verification

Schedule: Monday–Friday, 8:30 AM–4:30 PM
