Newhall Avenue – Your Bridge Between Hospital and Home
At Newhall Avenue, our Santa Clarita Post Acute Care Center is dedicated to providing a continuum of care that bridges the gap between hospital and home. We are seeking a compassionate and skilled Licensed Physical Therapist (PT) to join our experienced rehabilitation team.
About the RoleAs a Physical Therapist, you will work closely with our interdisciplinary team to provide high-quality, patient-centered rehabilitation services. You will evaluate residents, develop individualized plans of care, provide skilled physical therapy interventions, and help residents improve their strength, mobility, balance, safety, and overall functional independence.
ResponsibilitiesPerform initial evaluations and ongoing assessments to identify resident needs, functional limitations, and goals
Develop and implement individualized physical therapy plans of care
Provide skilled physical therapy interventions, including therapeutic exercise, balance training, transfer training, gait training, and mobility training
Monitor resident progress and modify treatment plans as clinically appropriate
Collaborate with physicians, nursing, occupational therapy, speech therapy, and other members of the interdisciplinary team
Communicate effectively with residents, families, caregivers, and healthcare professionals
Complete accurate, timely, and compliant clinical documentation
Participate in discharge planning and provide recommendations for safe transitions of care
Must be a currently licensed Physical Therapist (PT) in the State of California
Graduate of an accredited Physical Therapy program
New graduates are welcome and encouraged to apply
Skilled nursing or post-acute care experience preferred, but not required
Strong clinical judgment, communication, and interpersonal skills
Ability to work collaboratively as part of an interdisciplinary healthcare team
