This is a full-time role with an assigned East Bay territory, providing in-home physical therapy services to patients throughout the region.

About the Role

We are seeking compassionate and motivated Physical Therapist Assistants (PTAs) to join our growing home health team serving San Francisco and Peninsula communities.

Under the supervision of a licensed Physical Therapist, PTAs provide skilled therapy services to patients recovering from illness, injury, surgery, or functional decline in the home setting. The ideal candidate is patient-centered, collaborative, and committed to helping patients improve mobility, safety, strength, and independence.

Qualifications

• Current California Physical Therapist Assistant license required
• Graduate of an accredited Physical Therapist Assistant program
• Home health experience preferred, but not required
• Strong communication and documentation skills
• Ability to work collaboratively with patients, caregivers, physicians, and clinical teams
• Valid California Driver’s License required
• Multilingual abilities are strongly valued

Responsibilities

• Provide skilled physical therapy interventions under the direction and supervision of the Physical Therapist
• Implement individualized treatment plans established by the supervising PT and physician
• Assist patients in improving mobility, strength, balance, endurance, and functional independence
• Observe, document, and communicate patient progress and changes in condition
• Educate patients and caregivers on home exercise programs, safety, mobility strategies, and fall prevention
• Maintain accurate, timely, and compliant clinical documentation
• Participate in care planning discussions and case conferences
• Collaborate with physicians, nurses, caregivers, and other care team members to support coordinated patient care
• Support discharge planning and continuity of care
• Participate in quality improvement and continuing education initiatives

Pay Range

$45.00 – $65.00 per hour + Sign-On Bonus
