Responsibilities
- Deliver direct nursing care for a patient or assigned group in a home hospice setting according to established standards and the nursing process.
- Collect and analyze relevant data to determine diagnoses and care issues; develop and implement care plans to achieve outcomes.
- Supervise and coordinate activities of various levels of nursing staff; collaborate with multidisciplinary teammates.
- Coordinate care with patients, families, and the treatment team; provide education and learning opportunities.
- Participate in discharge planning to ensure continuity of care; document care plans, progress, and outcomes.
- Delegate appropriately, ensure patient safety, and maintain a safe, healing environment.

Requirements
- Current Registered Nurse (RN) license in the state of practice, or state of assignment
- BSN preferred
- At least two years of recent ICU/acute care experience
- BLS/CPR certification required; ACLS certification required per policy; HLFS required
- EPIC EMR experience required
- Ability to perform in a home healthcare – hospice setting; strong communication and teaming skills
