Position Title: Physical Therapist (PT) or Physical Therapist Assistant (PTA)
Department: Rehabilitation Services
Reports To: Director of Rehabilitation

Position Summary

The Physical Therapist (PT) or Physical Therapist Assistant (PTA) provides rehabilitative services to residents in a skilled nursing facility (SNF) to improve mobility, strength, balance, endurance, and overall functional independence. The PT/PTA works collaboratively with the interdisciplinary care team to develop and implement individualized treatment plans that enhance residents’ quality of life and support safe discharge planning.

Essential Duties and Responsibilities

Evaluate residents’ physical functioning and rehabilitation needs (PT only).

Develop and implement individualized treatment plans in accordance with physician orders and resident goals.

Provide skilled physical therapy treatments including:

Therapeutic exercise, Gait and transfer training,Balance and mobility training, Strengthening and endurance activities

Pain management techniques

Fall prevention interventions

Monitor resident progress and communicate changes or concerns to the rehabilitation team and nursing staff.

Document evaluations, treatments, progress notes, and discharge summaries accurately and timely in compliance with facility, Medicare, Medicaid, and state regulations.

Educate residents, families, and caregivers on therapy plans, home exercises, and safety recommendations.

Participate in interdisciplinary care plan meetings and discharge planning.

Follow infection control, HIPAA, and facility safety procedures.

Qualifications

Graduate of an accredited Physical Therapy or Physical Therapist Assistant program.

Current state license as a Physical Therapist or Physical Therapist Assistant.

CPR certification preferred. Experience in skilled nursing, rehabilitation, or geriatrics preferred.

Ability to work effectively as part of an interdisciplinary team.

Compassionate and resident-centered approach to care.

Physical Requirements

Ability to lift, transfer, and assist residents with mobility activities.

Frequent standing, walking, bending, reaching, and repositioning residents.

Ability to safely operate therapy equipment and respond to emergency situations.
