Fairview Skilled Nursing Home is seeking a detail-oriented MDS Assessor RN to complete resident assessments, ensure accurate MDS documentation, support regulatory compliance, and contribute to quality resident care.

• Complete MDS assessments accurately and on time.

• Gather assessment information from residents, families, and the interdisciplinary team.

• Ensure documentation supports MDS coding and regulatory requirements.

• Collaborate with nursing, therapy, social work, and other departments.

• Maintain compliance with CMS, Medicare, Medicaid, and PDPM guidelines.

• Assist with care planning and quality improvement initiatives.

• Current New York State RN license.

• Skilled nursing or long-term care experience preferred.

• Knowledge of MDS 3.0, PDPM, Medicare, and Medicaid.

• Strong assessment, organizational, and communication skills.
