Are you passionate about helping older adults maintain their independence and improve their quality of life? Do you thrive in a collaborative, interdisciplinary care environment? PACE North is looking for a dedicated and compassionate Physical Therapist to lead and deliver high-quality care to the aging population - both at our Center and in participants' homes.
We are seeking a licensed Physical Therapist to join our team on a temporary, fixed-term basis to provide coverage during a maternity leave.
Position Details
- Temporary / Fixed-Term
- Approximate duration: 4 months
- Schedule: Full-Time, hours negotiable, Monday - Friday
What You’ll Do:
As a Physical Therapist at PACE North, you’ll be responsible for helping participants maximize their mobility and independence. You’ll:
- Conduct initial and periodic assessments of participants’ physical and functional status
- Develop personalized care and treatment plans, including therapeutic exercise, gait training, and mobility support
- Collaborate with an interdisciplinary care team and communicate changes in participant condition
- Recommend and evaluate the need for durable medical equipment
- Train family members, caregivers, and staff on implementing therapy strategies
- Contribute to participant discharge planning and continuity of care across settings

What We’re Looking For:
Education
- Bachelor’s in physical therapy, Master’s, or Doctorate
Licensure/Certifications:
- Licensed Physical Therapist in Michigan
- CPR Certification (We can train)
- Valid Michigan Driver’s License
Skills & Qualities:
- Strong clinical competency in geriatric care
- Basic computer proficiency
- Ability to work independently and collaboratively
- Clear communication and documentation skills
- Commitment to participant-centered care and quality improvement
