Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and document patients’ vital signs, health status, and behavioral changes with attention to detail and accuracy
- Support medication administration following proper protocols and HIPAA regulations to ensure safety and compliance
- Prepare nutritious meals tailored to individual dietary needs while maintaining cleanliness in the kitchen area
- Conduct patient observation to identify changes in condition or behavior, reporting concerns promptly to your supervisor
- Maintain a clean, safe environment by performing routine cleaning tasks and adhering to infection control standards
- Support residents’ participation in social activities and community outings to promote engagement and quality of life
- Follow individualized care plans developed by healthcare professionals to meet each client’s unique needs
- Document all care activities thoroughly in compliance with HIPAA and organizational policies
Qualifications
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred
- Knowledge of patient care techniques including CPR, First Aid, person-centered care, trauma informed care, and therapeutic language.
- Strong observational skills for monitoring patient health and behavior changes
- Familiarity with medication administration procedures and vital signs measurement
- Compassionate attitude with excellent communication skills for working effectively with clients and team members
- Valid certifications in CPR and First Aid; additional certifications in HIPAA compliance, ProAct is highly valued
- Experience working with individuals with disabilities or developmental delays is highly valued
