Position OverviewThe Geriatrics & Extended Care Service at our healthcare system is seeking a skilled and enthusiastic Geriatrics Board Certified Attending Physician for the role of Medical Director of the Community Living Center (CLC). This position provides an opportunity to serve our nation's veterans while enjoying a commendable work-life balance. The CLC offers nursing home-level care for veterans requiring short-term rehabilitation, skilled nursing, long-term care, dementia care, or hospice services.
Key ResponsibilitiesThe Medical Director of the Community Living Center will:
- Deliver patient care directly or oversee care provided by CLC-assigned providers and medical trainees in both inpatient and outpatient environments.
- Provide care for adults with both acute and chronic medical conditions across a range of CLC services, including skilled nursing, long-term care, hospice/palliative care, pain management, and rehabilitation.
- Assess and manage adults with single or multiple disorders across various subspecialties, focusing particularly on geriatric medicine.
- Collaborate effectively within interdisciplinary teams to ensure comprehensive patient care.
The Physician in the Community Living Center will also be accountable for the overall quality of medical care provided to the veteran population. This role involves maximizing the efficient use of available resources to treat the assigned populations.
Clinical DutiesThe Physician will have the following direct clinical responsibilities:
- Review patient histories, conduct examinations, and develop assessments and care plans for CLC residents, coordinating evaluations, diagnostic testing, clinical follow-ups, and care coordination.
- Oversee the day-to-day management of veterans in collaboration with the Community Living Center team.
- Foster an environment of efficiency and accuracy in the timely submission of completed documentation.
- Work alongside Nurse Practitioners, Physician Assistants, ancillary healthcare providers, and various clinical specialties to facilitate hospital admissions, post-discharge care, and specialty consultations as necessary.
- Participate in the screening of veterans and the completion of consults for the CLC.
- Be part of the on-call schedule for telephonic coverage addressing urgent needs of veterans in the CLC after administrative hours.
- Engage in interdisciplinary team meetings.
- Take on additional clinical duties as assigned by the Associate Chief of Staff for Geriatrics & Extended Care.

- CME: Potential reimbursement of up to $1,000 per year (must be full-time with board certification).

- Contract: No physician employment contract and no significant restrictions on moonlighting.
- Education Debt Reduction Program (EDRP): Eligible participants may receive up to $200,000 towards qualified loans over five years (not exceeding $40,000 per year).
Preferred candidates will have:
- Board Certification with the American Board of Internal Medicine (ABIM), American Board of Family Medicine, or Board Certification in Geriatric Medicine.
- Experience in geriatrics or a fellowship, with a passion for working within a multidisciplinary team.
