Your Responsibilities
Deliver comprehensive primary care to adult and elderly Veterans in an outpatient environment, emphasizing continuity and holistic care.
Lead a collaborative PACT team (including RN, LPN, clerical staff, and support personnel) to address chronic illnesses, preventive care, and urgent health issues.
Utilize evidence-based practices with access to on-site specialists, mental health services, and additional resources within the VA framework.
