The Sub-Acute and Long-Term Care sector is rapidly growing, offering excellent opportunities and a flexible work-life balance (no on-call duties or weekend shifts)! We are currently looking for a Full-Time provider (40 hours per week) for our Cedar Ridge Center situated in Sissonville, WV. This role is open to either a nurse practitioner or a physician assistant.
The annual base salary for this position ranges from $104,000 to $120,000, along with a no-cap incentive compensation bonus program.

Key ResponsibilitiesThe Advanced Practice Provider will report directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves collaborating on-site with the Center Leadership Team to meet specific goals and priorities. Additionally, the Advanced Practice Provider will work closely with the Medical Director and/or Attending Physician to deliver patient-specific medical care.
Responsibilities include:
- Providing primary and urgent care services to residents of the nursing center in partnership with other medical providers.
- Working alongside Center Leadership to fulfill our mission of enhancing lives through high-quality healthcare and compassionate service.
The ideal candidate must have graduated from an accredited School of Nursing and hold a valid registered nurse license. For nurse practitioners, certification is required from an accredited Nurse Practitioner Program with a Master's Degree in Nursing, specializing in Older Adult or Family Nurse Practitioner (FNP). Other specialties may be considered based on experience and organizational needs. For physician assistants, graduation from an ARC-PA accredited program and successful completion of the PA National Certifying Exam (PANCE) is necessary, along with state licensure and NCCPA certification.
Preferred qualifications include:
- A minimum of two years of experience working with adult or geriatric populations.
- In states where permitted, an unrestricted professional license and DEA for APRNs and PAs.
This position is designed for a full-time commitment, ensuring a stable work environment with no on-call requirements or weekend shifts.

Salary Range: USD $104,000.00 - USD $120,000.00 per year.
