The Sub-Acute and Long-Term Care sector is rapidly growing, offering excellent opportunities and a flexible work-life balance (no on-call duties or weekend shifts)! We are currently looking for a full-time nurse practitioner to join our team at a Skilled Nursing and Rehabilitation Center located in Jersey Shore, PA, with a commitment of 40 hours per week.

The annual salary for this role ranges from $110,000 to $130,000.

Key Responsibilities

The Advanced Practice Provider will report to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves collaborating on-site with the Center Leadership Team to meet specific center goals and priorities. Additionally, the Advanced Practice Provider will work closely with the Medical Director and/or Attending Physician to deliver patient-centered medical care.

Responsibilities include:

  • Providing primary and urgent care services to residents of the nursing center in partnership with other medical professionals.
  • Working alongside Center Leadership to fulfill our mission of enhancing lives through high-quality healthcare and compassionate service.
Qualifications

The ideal candidate must have graduated from an accredited nursing program and hold a valid registered nurse license. Certification as a Nurse Practitioner is required, with a Master’s Degree in Nursing and a focus on Older Adult or Family Nurse Practitioner (FNP) specialties. Other specialties may be considered based on experience and organizational needs.

Preferred qualifications include:

  • A minimum of two years of experience working with adult or geriatric populations.
  • In states where permitted, an unrestricted professional license and DEA registration for each state of practice.

Salary Range: $110,000 - $130,000 per year.
