Job Opportunity: Physical Therapist

Our organization is in search of a committed Physical Therapist to become a vital member of our Healthcare & Medical Services team. This role focuses on delivering compassionate, evidence-based rehabilitation services primarily to senior residents within our facility.

Key Responsibilities
  • Conduct thorough evaluations of patients to assess their physical therapy needs.
  • Design personalized treatment plans aimed at restoring function, alleviating pain, and enhancing independence and quality of life.
  • Implement skilled interventions, including therapeutic exercise programs, manual therapy, gait and balance training, and appropriate modalities.
  • Collaborate closely with an interdisciplinary team, which includes nursing, occupational therapy, speech therapy, social services, and physicians, to ensure comprehensive, patient-centered care.
  • Regularly document patient progress in the electronic medical record system.
  • Communicate effectively with patients and their families regarding treatment plans and home exercise regimens.
  • Participate in care plan meetings and discharge planning to facilitate safe transitions of care.
Requirements and Qualifications
  • Valid physical therapy license.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively in a team-oriented environment.
  • Experience in geriatric and post-acute rehabilitation is preferred.
