Responsibilities
- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
- Administer medications and monitor patients for side effects or reactions
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
- Monitor and record patient vital signs, symptoms, and behaviors.
- Assist with patient mobility, hygiene, and comfort.
- Educate patients and families on health maintenance and disease prevention.
- Collaborate with healthcare teams to develop and implement care plans.
- Assist with medical procedures and provide post-procedure care
- Maintain accurate patient records and documentation in accordance with facility standards.
- Ensure compliance with healthcare regulations and standards of care is provided
- Currently licensed Registered Nurse RN or Licensed Vocational Nurse LVN in state of Texas or compass state
- Previous experience as a Registered Nurse or Licensed Vocational Nurse is preferred
- Excellent interpersonal and client care skills
- Valid drivers license
- First aid CPR certification required
- Compassionate, patient-focused approach to care
- 1 year Experience in long-term care, acute care, rehabilitation or Home Health for Pediatrics preferred
