Job Summary:

Key Responsibilities:

Provide personal care services including bathing, grooming, dressing, and toileting - Assist patients with mobility, transfers, and ambulation
- Monitor and report changes in patient condition to supervising nurse
- Assist with vital signs (temperature, pulse, respiration, blood pressure)
- Support patients with activities of daily living (ADLs)
- Maintain a clean, safe, and comfortable environment for patients
- Follow care plans as directed by the RN/LVN
- Document care provided accurately and timely
- Comply with all agency policies, HIPAA, and safety standards
Education & Certification Requirements:

Must be a Certified Nurse Aide (CNA) with a valid CNA license in the state of Texas (or applicable state) - High School Diploma or GED preferred
- Current CPR certification (preferred or required depending on agency policy)
Experience Requirements:
-
Prior experience in Home Health, Hospice, Nursing Home, or Hospital setting preferred - Experience working with elderly, disabled, or terminally ill patients strongly preferred
- Familiarity with ADLs and patient care documentation
Skills & Competencies:

Strong communication and interpersonal skills - Compassionate and patient-centered attitude
- Ability to work independently in patient homes
- Reliable transportation and valid drivers license (if required for field visits)
- Strong attention to detail and documentation accuracy
