Responsibilities
- Perform comprehensive patient assessments and develop individualized plans of care
- Provide skilled nursing services in accordance with physician orders and agency policies
- Manage and coordinate care for home health and hospice patients
- Monitor patient progress and adjust care plans as appropriate
- Educate patients and families on disease management, medications, and end-of-life care
- Accurately document visits and maintain compliance with Medicare regulations
- Collaborate with physicians and interdisciplinary team members
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid drivers license
- First aid/CPR certification required
