Details are listed below.Practice Setting: 4 days outpatient and 1:3 cover Reason for Coverage: 3 month maternity leaveDates of Coverage: Sept-Dec 2026Schedule: M-W-TH-FRi outpt/8:30-5pm/ and call Call Schedule: 1:3 -evening every night//weekend Friday 4pm-Mon 7AMCall Ratio: 1:3Patient Population for Required Coverage: Adult Geriatric OBWhat Percentage of the Practice is Hematology Oncology Internal Medicine ?: Hem -60Onc-40IM -0Number of Physicians in Practice: 3 --one having babyWill There be Radiation Oncologists Present During Locum Tenens Coverage? yesHow Many APPs Provide Coverage: 2 Average Number of Patients Per Day - OUTPATIENT Clinic/Office: 15-16Average Number of Patients Per Day - HOSPITAL Call/Rounding: Approx 1-5 Clinical Area: Oncology - Outpatient Setting Hematology - Outpatient Setting Oncology - Inpatient Setting Hematology - Inpatient SettingTrauma Level/Designation Hospital: 2 level Documentation System/EMR: EPICAdministration of Chemotherapy: Nurse oncologistEstimated Credentialing Timeframe: TBD-we have approval for expedited start if necessaryProvider Must be local - only mileage is reimbursedRequired Duties: Participation on a Tumor BoardOrders TestsOrders ImagesPrescribesRates to be submitted:Malpractice rates must be included in ratesOrientationHourlyHoliday HourlyNight call with minimum of 2 hours gratisHoliday Night call with minimum of 2 hours gratisConversion Fee: 25kTier 1: Hourly: 700 and below Night Call: 575 and below Callback: 750 and belowTier 2: Hourly: 701-720 Night Call: 576-600 Callback: 751-800**Trinity Health does not reimburse for credentialing costs or application fees.**
