RN Case Manager - $10K Sign-On Bonus

Position: RN Case Manager | Day Shift | Up to $10K Sign-On Bonus + Relocation
Location: Fort Walton Beach, FL
Type: Full Time / Permanent
Salary Range: $34 - $47 per hour (Based on experience)
Sign-on bonus up to $10K + Relocation

Shift: Days | 8:00 AM – 4:30 PM | Rotating Weekends

Key Requirements:
• Active RN License
• Associate Degree or Certificate required
• Minimum 2 years of recent Acute Care experience
• Background in ICU, CCU, Med-Surg, LTAC, or Case Management preferred

Key Responsibilities:
• Coordinate patient care and discharge planning
• Work closely with physicians, nurses, and interdisciplinary teams
• Educate patients and families on treatment and care plans
• Ensure safe patient transitions and continuity of care
