Brite Horizon is seeking a motivated and relationship-driven professional to support client intake and build strong connections with community partners.
This role is key to helping individuals access quality services by coordinating referrals, communicating with case managers and county boards, and ensuring a smooth onboarding experience.
What You’ll Do
- Serve as the primary point of contact for:
- County Boards (DODD)
- Service & Support Administrators (SSAs)
- Case managers
- Respond to new referrals and service inquiries
- Coordinate intake process for new clients
- Gather and review required documents (ISP, assessments, etc.)
- Communicate with families and stakeholders
- Match clients with appropriate services and staffing
- Maintain ongoing relationships with referral sources
- Track outreach, referrals, and follow-ups
- Support growth by identifying new referral opportunities
What We’re Looking For
- Strong communication and relationship-building skills
- Organized and detail-oriented
- Comfortable with phone, email, and follow-up communication
- Ability to manage multiple referrals and timelines
- Professional and responsive
Preferred Experience
- Experience in:
- DODD services
- home care / PASSPORT
- case management
- intake coordination
- Familiarity with:
- ISP (Individual Service Plans)
- waiver services
- working with county boards or healthcare systems
